Heavy is strong but not that physically fast. He remembered fight with the Scout and was afraid the same could happen here. He requested a smaller arena, where fight would be close and personal. Heavy was not prepared to hear “no“ and Medic knew that. But this would be unfair towards other fighter. So Medic allowed Demoman to use any found object as a weapon. But for one attack only.
