The road was built in 1959-1966, when it was two-lane. In 1990, a duplicate section of the M-4 appeared in the Domodedovo district of the Moscow region, in November 1998, the first toll section in Russia was opened to bypass the village of Khlevnoye. In the 2000s, oncoming traffic was divided on the highway, multi-level interchanges appeared, and the length of sections with night lighting increased.
In 2010, the road came under the control of the state-owned Avtodor company. At that time, more than half of the area needed major repairs. As the sites were being renovated, the company began to charge tolls.
In 2011, the M-4 became a four-lane highway all the way from Moscow to Voronezh. Later, sections bypassing Voronezh and Pavlovsk, the villages of Novaya Usman, Rogachevka and Losevo were put into operation.
