He can’t kill her because he has feelings for her, so he enlists the help of Castiel and Rowena (off screen) to change Josie’s fate which leads to her having been born during modern times instead. When they meet this second time, she’s a regular siphoner witch who joins him and Sam on several of their hunts. Because she’s no longer a heretic, the merge looms over their heads but is avoided when they go after the Tribrid. Dean blames himself for what happened and decides to set things right by undoing what he did and going back to the 1800s where he meets Josie. She turns him into a vampire and then he’s brought back to modern times. Then in order to prevent her staying dark, he goes back in time to the 1920s where they meet again. This time he helps her turn her humanity on so by modern times she helps Dean by giving him her blood to turn him back into a human. Later he gets into a car accident and ends up in a coma. Josie then does a ritual to turn him back into a vampire, but magic always has a cost. Dean loses his humanity and because he knows he’s no match for a heretic, he allows Michael to use his body (but Dean is in control while Michael is a prisoner). From there I think it’s pretty self-explanatory. Also, Josie kept all the darkness that causes her to turn off her humanity in the hourglass that she then breaks. As for the end, it’s open ended.
